BTC is a currency. Like any other.
You pay your dollars, pounds, whatever in exchange for bitcoins.
Then if you want to exchange your bitcoins for your own currency again, you have to convince someone or a company to buy them off of you.
Where that money goes is up to you. Someone could give it via paypal, via cheque, cash in hand, bank transfer, whatever you both agree on really.
