In many contexts, knowing the year isn't critical. For these situations, I've adopted the system of writing the shortname of the month. So "3/4" which can be ambiguous becomes either "Mar 4" or "Apr 3". In contexts where the year is important, I always write in ISO format.

For formal documents, you always write the date out in full. Its mostly in internal documents that you'll see things like "02/27/13", but never in my professional life have I ever seen "27th of February, 2013" except in legal situations. But it is always in a sentence, like "...on the 27th of February, 2013, the defendant...". You'd never see it written like that on a letterhead or anything though.
